Ulta Lab Tests has the most extensive selection of fertility- and pregnancy-related tests of all of the providers we evaluated, coming in at 114 different services at the time of this review. You can either search for the name of the test, if you know exactly what you need, or click on one of the subcategories (such as Infertility for Men, Infertility for Women, Reproductive Hormone Tests, and so on). The list is too long for us to tell you every option at Ulta, but here's a sampler of what you'll find there:

  • Infertility for Men: 17 tests, including Post-Vasectomy Sperm Count ($59.95) and Testosterone: Free/Bioavailable/Total, LC/MS/MS ($90.95)

Save as much as 80% over typical self-pay costs

You'll notice pretty quickly that Ulta Lab Tests is significantly less expensive than many of the fertility testing providers we researched. The company says that's from their negotiations with labs to bring costs down as much as 80% when compared with the self-pay price you'd be charged if you went to the lab directly. There's also a good chance that you can get your Ulta Lab fertility tests even cheaper: just watch for a pop-up as you browse. During our most recent visit to the site, we got a discount of 20% by providing our email address; the code was displayed on the screen immediately, with no waiting for an email to arrive.

Expect draw fees of $8.95+

How will you submit your samples for Ulta Lab Tests? They refer to their partner locations as "Patient Service Centers”, but their FAQ reveals that those will almost always be Quest Diagnostics locations. You can use the Locations tab to find the options near you; there's usually an $8.95 "draw fee” for having your blood sample taken, but some locations come with a "premium draw fee” and may even be mobile for the purpose of having your draw at home or another location of your choice. We saw those fees for anywhere between $45 to $100. You'll select a testing location during checkout and pay those fees in advance.

Once your samples are sent to one of Ulta Lab Tests' national CLIA-certified laboratories, your results should be available in your online patient portal within 1-3 business days. While your fertility testing doesn't come with a physician consultation of any kind, you will be alerted if your results show anything abnormal with a "critical or panic value” that requires immediate medical care, or if your tests are positive for a communicable disease.

Excellent reputation, loyal repeat customers

Ulta Lab Tests has a great reputation overall. The Better Business Bureau offers the company a perfect "A+” rating, and customers have left numerous 5-star reviews for the service (or 10-star reviews, if you look at the ones featured on the Ulta Lab Tests site itself). We're particularly encouraged by those who have used Ulta time and again because the tests are consistently affordable and the results are accurate. While there aren't any reviews on the specific fertility tests themselves, we have every reason to believe that the patient experience for those analyses is just as top-notch.

Walk-In Lab isn't the easiest site to use for fertility testing: they don't offer that as a distinct testing category, so you'll have to do some searching to find the analyses you need. For example, you could do a generic search for "fertility” (119 results), or use the filters/categories for Women Specific Tests, Men Specific Tests, and Hormone Tests.

But, once you do find the test(s) that you need, it's pretty easy to see where you'll go to have the samples drawn: most will be offered either through Quest or Labcorp, though we found a few tests through other providers (ZRT, GDX). We like that Walk-In Lab puts that decision in your hands, and that they make it clear what you'll pay upfront. While we couldn't find more details about the 110% price match guarantee advertised on the HealthLabs site, we'd encourage you to keep that in mind if you happen to find one of their fertility tests at a lower price somewhere else. This company has a toll-free number for you to reach out, to ask about the price guarantee or any other question you might have before or after doing your fertility test. If your test results warrant it, you can consult one of HealthLabs' physicians too. Most tests should have results to you within 3 days, and HealthLabs' Care Advisors are available every day from 6a-10p (CST).

Lots of 5-star reviews

With respect to overall reputation, HealthLabs is doing pretty well. Its parent company, Analyte Health, has an "A” rating and accreditation from the Better Business Bureau. Both on the BBB site and on third-party rating platforms, HealthLabs gets thousands of 5-star reviews. While you'll find the occasional glitch with getting tests processed or other customer service issues, HealthLabs' reps are always responsive. And, the vast majority of users say that the service is easy and fast, both for ordering the tests and getting them done with quick results.

HealthLabs earns a well-above-average rating from us. While we'd like to see some options for fertility testing panels, so that patients could more easily screen for multiple hormones at the same time, the a la carte pricing here is competitive even before taking advantage of the 110% price match guarantee. Ro is a direct-to-consumer telehealth company that specializes in providing personalized healthcare solutions through its digital platform. Founded in 2017, Ro initially gained recognition with its men's health brand, Roman, focusing on erectile dysfunction, hair loss, and other male health issues. The company has since expanded its offerings to include women's health through its Rory brand and acquisition of Modern Fertility, and general health through its Ro Pharmacy, which delivers prescription medications at low costs. Test options are more limited here

With respect to fertility testing, Ro's options are more limited in number than some options on our list. From what we can tell, they used to offer sperm testing but don't any longer; you can still log in to your dashboard if you've used that service previously, but there's no product listing for new clients of that type of fertility testing. You can also buy ovulation and pregnancy testing through Ro, but the main product you'll probably be interested in is the Modern Fertility Hormone Test.

Your birth control method will determine your fertility testing here

This fertility test for women claims to be "the same hormone test typically offered by reproductive endocrinologists for a fraction of the price”. What's tested will depend on whether or not you're using hormone-based birth control: all patients' blood samples will be analyzed for AMH and TSH, while only those who aren't on hormonal birth control may also be tested for FSH, Estradiol, Prolactin, Free Thyroxine (depending on your levels of TSH), and LH. At a price of $179, we're not sure that Ro's fertility test is quite the bargain they advertise (given that it's the same cost whether you only test for AMH/TSH or all 7 elements), but it's not too far off the mark compared with other testing options on our list.

Very thorough help after you test

But, we're big fans of Ro's transparency with what you'll get as a user of their Modern Fertility Hormone Test. Check out their sample fertility hormone test report to get an idea of what exactly you'll learn when you opt for their service. The company seems to go to great lengths to make sure you understand your results (which you should have within 7-10 days of your sample arriving at their lab; you'll either do the finger prick yourself or visit a Quest lab to have it drawn). And, once your results are in, you'll be able to use Ro's customized fertility planning tools based on your goals and timeframe, plus a community platform to ask questions of their fertility experts and connect with others who understand your journey.

"A+” and accreditation from the BBB

Ro has an "A+” rating and accreditation from the Better Business Bureau. We did find it a little funny that the BBB's description of Ro said that it's "an online portal [for] male customers” given that the site is very obviously useful for males and females, but okay. Evidently the BBB hasn't caught up with Ro's 2021 acquisition of Modern Fertility.

And the rest of Ro's reputation? To be honest, it depends on what you're coming to Ro for. If it's for the Modern Fertility Hormone Test, you're probably going to be happy with the service: across 2,200+ reviews (curated/filtered, we presume), the average is an impressive 4.5 stars. Although those reviews didn't go past 2023, for reasons we're unsure of, they're mostly enthusiastic recommendations for this fertility test. Some said they would have preferred to have it done at their doctor's office for the cost of a copay, but not everyone has access to insurance; that's why companies like Ro exist, after all.

Non-fertility customers aren't always thrilled

Other Ro services are the reason behind the company's 3.9-star average at Trustpilot, most recently their semaglutide injections. That service gets a lot of 1-star reviews. But, when we filtered Trustpilot's ratings for the word "fertility”, there were only two matches, and neither would impact your decision on whether or not the Modern Fertility Hormone test is right for you.

Solid option for comprehensive female fertility testing

If you're specifically looking for female fertility testing for the purpose of having a baby (whether now or sometime in the future), Ro's singular option is a solid one. The interface is modern and provides you with a lot of tools for your planning, and it's one of the only ones we've seen that makes a point to find out more about you (if you're on hormonal birth control) to guide you to the right fertility test panel. While we'd love to see some options for men (back) on the table and maybe some lower-cost tests for women who just need to analyze one or two aspects of their fertility, Ro's overall value is good.

All testing done via Labcorp or Quest

All of the fertility tests offered by Personalabs will have the samples collected either at Quest or Labcorp: some tests give you the choice of which one to use, but a few are limited strictly to one or the other (an hCG Pregnancy Test with Gestational Table blood test is only offered via Quest). None of the fertility tests here have an at-home sample collection alternative, so keep that in mind if you were hoping for fertility testing you could do in the privacy of your own home.

Easy to buy your test and go to the lab the same day

Here's how it works: first, you'll create a Personalabs account and choose the fertility tests you want (and through which lab; you're able to mix-and-match if you have both Quest and Labcorp conveniently located near you). Once you've paid for your tests, Personalabs' physician network will approve the order, usually within 2-4 hours, though some tests can take up to 24 hours to be approved. Visit the lab at your convenience; you may want to schedule an appointment online through the lab's website. You should have your results within 24-48 hours.

Can pay for a telehealth consultation to go over results

One extra perk offered by Personalabs is the option of scheduling a follow-up with one of their board-certified physicians to go over your results. These are all telehealth-type appointments, so you can "meet” with the doctor in the privacy of your own home. You'll have to check with Personalabs on pricing, but they say that they're usually less expensive than a face-to-face visit with your usual doctor.

Given that both Quest and Labcorp now have the ability to order fertility testing directly from them, why would you go through Personalabs at all? The most obvious answer is price: you could pay less for the exact same test when you start here. For example, the Anti-Mullerian Hormone (AMH) Blood Test is offered through Personalabs for $98 at Quest and $112 at Labcorp; when you go directly to either of those labs, Quest charges $135 and Labcorp doesn't even have it listed.

Overall, the reputation for Personalabs is good, shown by an "A+” from the Better Business Bureau and plenty of positive reviews from customers reporting back in various places. Most people say that they got their results as expected and there were no surprises during the process, though we couldn't find any comments or reviews specific to fertility testing through Personalabs.

A few customer service snags

Just one tip: don't count on getting answers to any questions through the site's live chat. Even when we visited during the regular posted business hours, we were told that their agents were "away” and invited to leave a message; the link to do so resulted in a 404 error (which is less than impressive customer service, to be honest).

We give Personalabs an above-average score among the options for fertility testing on demand. Although this service is missing some of the convenient test-at-home kits you'll see elsewhere, we appreciate the way that Personalabs makes it easy to get your samples collected through both the Quest and Labcorp networks, often at prices that beat going directly to those labs themselves. Since 2015, Everlywell has been on a mission to put healthcare in the hands of consumers by offering easy access to a wide variety of lab tests. With no doctors' orders required, these tests can be purchased at-will. That's a big win for fertility testing, especially when it's a sensitive subject you might not want to have to schedule an in-person visit with your doctor to discuss just yet. Everlywell has been featured by Bloomberg, Cosmopolitan, Forbes, NYT, Fast Company, and Time, so it's not exactly a new kid on the block. Everlywell is also the testing service behind Natalist, another name you may come across as you search for on-demand fertility tests.

One fertility test offered

But, we want to start right off the bat and point out that there's only one option for fertility testing at Everlywell: the Women's Fertility Test. It comes as a fully at-home kit (no lab visit required) and will use both saliva and a finger prick to test levels of estradiol, LH, FSH, TSH, and total testosterone. The kit comes with clear instructions for how (and when) to collect each component of the testing; just don't forget to register your kit using the QR code, so that you'll be able to easily access your results. Be aware that you may have to do some of the tests during different parts of your menstrual cycle; it's not a "get it all done in one day” kind of fertility test here.

Your kit will be sent to a CLIA-certified laboratory, and a board-certified physician will review the results. You should have them delivered to your Everlywell dashboard within 5 business days of the lab receiving your kit. You'll pay $149 for the Women's Fertility Test, but there's an easy way to save a bit: we thought it was a little odd for a fertility test provider to offer a "spin to win” discount, but when every penny matters, why not? By entering your email address, you could get free shipping on your order (though we're pretty sure every order qualifies for that already), a 10% discount, or a savings of $5 to $10 on your first order.

Good overall ratings on the fertility test

Across 40 reviews left on the Everlywell site for the Women's Fertility Test in particular, the ratings averaged 4.58 out of 5 stars and 95% would recommend it to a friend. Users described the instructions as easy to follow and said that the results helped them decide on the appropriate next steps for their healthcare. One woman was unhappy with results taking longer to arrive than she had expected, and another complained that her PCP referred her for a new blood test when the results from Everlywell seemed far from accurate. On the whole, though, this fertility test seems to do what it promises for most patients.

No tests for men

Those are all of the positives of using Everlywell for fertility testing. Unfortunately, there are quite a few negatives you should be aware of too. First off, the most obvious drawback: there are zero options for male fertility testing here. You're just flat out of luck if you're looking for problems on the XY side of the equation.

Overall reputation isn't so fantastic

Second, and maybe even more importantly, Everlywell's overall reputation has us concerned. Even though the Better Business Bureau has given this company an "A+” rating, the reviews and complaints left both with the BBB and on places like Trustpilot paint a picture we can't get excited about. Everlywell has a membership-based structure; while you should be able to order one-off tests without subscribing, many people have complained about winding up with a membership even though they didn't want to opt in. Others have received tests they never ordered (and were charged for them), and across the small spectrum of tests that Everlywell offers, there have been lots of issues with getting the tests processed or receiving results that later turned out to be inaccurate. With customer ratings averaging below 2 stars everywhere that we looked, this company isn't doing a fantastic job of keeping people happy, despite the high ratings on the women's fertility test itself.

Too much room for improvement

While Everlywell looks like a good option for fertility testing at first, there are some downsides that are hard to overlook. You've got much more limited options than you'll find with many of the test providers on our list (nothing for men at all), and we're not fully confident that the results you'll get with the women's fertility test here are going to be on time and accurate. We'd definitely recommend looking at the higher-ranking companies on our list, and leave Everlywell as a last resort.

myLAB Box has been around since 2014, providing tests for a wide range of needs that can all be done at home: no lab visit required, even for blood sample tests. You may have even seen some of their test kits sold at your favorite supermarket or drugstore, but, of course, their best selection is always going to be through their online storefront. That includes fertility testing.

Several fertility tests for men and women

myLAB Box offers the following fertility tests, all under the banner of "Family Planning”:

  • Male Fertility Test ($89): this 2-test kit lets you test your sperm count at home. While they don't go into details, myLAB Box explains that as part of this test, you'll have a recorded video of your sample inside their app. But, the phones with which they said the app is compatible were pretty out of date (going only back to the Google Pixel 2XL, which was many years old at the time of our review).
  • Perimenopause Test ($99): requires an at-home finger prick to test FSH, Estradiol, and Progesterone.
  • Ovulation Confirmation Test ($32): includes 5 test strips to measure PdG in urine. No need to send any samples in for testing; all done at home.
  • Testosterone Test ($79): this kit is available for both males and females; just be sure to select the desired option. We were confused, however, that in one part the description says this is done by a finger prick; in another, it says the test is conducted via saliva sample.
  • Female Fertility Test ($149): this test measures Estradiol, FSH, LH, and Testosterone with a finger prick and saliva sample.
  • Ovarian Reserve (FSH) ($99): if you just want to test FSH and not the other components in the broader Female Fertility Test, this is the right option.
  • Postmenopause Test ($99): measures Estradiol and Progesterone. You may just want to do the Perimenopause test, though: it includes FSH at no extra cost.
  • Women's Health ($249): the most comprehensive of myLAB Box's fertility tests, it measures Cortisol, DHEA, Estradiol, FSH, LH, Progesterone, Testosterone, TPO, TSH, T3, T4 with a finger prick and saliva sample.
  • Men's Health ($199): covers a range of wellness components for men, including Estradiol, DHEA, Cortisol, and Testosterone through a finger prick.

Results timeframe depends on the test

How soon can you expect your results? As you might have concluded already, that depends on the fertility test you've chosen: some provide instant results, while others have to be sent to myLAB Box's partner labs, which are all CAP- and CLIA-certified. The company says you can expect to get results from the latter within 1-5 days of receiving your samples.

Not as transparent as we'd like

We do wish, though, that myLAB Box was more transparent with details about where the samples are going and the name/location of the lab(s); all we could determine is that it's likely in Florida, because the FAQ says that positive test results for chlamydia, gonorrhea, or trich are required by law to be reported to the FL State Department of Health. Beyond STDs, though, myLAB Box doesn't appear to provide any kind of physician consultation once the results are in. You would need to take your fertility test results to your own doctor (which isn't a bad idea anyway).

Too many people are unhappy with this company

Besides the vagueness of where the test samples are going and the lab that's going to test them, myLAB Box fails to keep up with the competition in one more big way: reputation. The Better Business Bureau gives myLAB Box a failing grade, and reviews left there average just 1 star out of 5. We found tons of complaints about results that never came, kits that weren't delivered even close to promptly, and having trouble getting help from customer service.

Below average for fertility testing

While we're absolutely here for fertility testing that's all done at home, we've got too many concerns about myLAB Box to give it anything more than a "below average” rating. (We're particularly spooked by the "video results” of the sperm testing!) Also, Quest has a good mix of fertility tests that can be done at home or through a blood draw at one of their 6,000+ locations nationwide. One bonus feature offered by Quest is mobile collection. If the service is offered in your area, you can pay a $55 fee to have a phlebotomist come to you and take your blood sample.

Test purchase includes doctor consultation (if desired)

Regardless of the collection route you choose, your results will be available on your Quest patient portal within 5 business days of your samples being submitted to the lab. If your results are outside of the expected range, you may get an alert call from an independent healthcare coordinator through PWNHealth. A consultation is included in your fertility test purchase from Quest (even if your results are within range), but you're encouraged to also share your results with your own physician if you choose.

Shop around and look for sales

Pricing is competitive at Quest, and they often have promotions that bring the costs down. That includes fertility testing, especially during seasons like Mother's Day and Father's Day (a little on the nose, but we'll take it). You'll still want to shop around a bit, though: with some rivals on our list who use Quest as one of their partner facilities for the sample collection, we did find fertility testing at a lower cost.

All of that sounds pretty great, right? Are you now wondering why Quest's fertility testing winds up so low on our list? It's all about the reputation. First off, there's no function on Quest's site for clients to leave feedback. In a way, we kind of understand: it seems like it might violate some kind of HIPAA law or something similar if patients were leaving comments on the tests they purchased; and yet, rival sites often allow people to leave semi- or fully-anonymous feedback.

"F” from the BBB and other reputation woes

Those kinds of comments could have helped tip the scales back in Quest's favor: a boost this company needs, when you look at the "F” rating it receives from the Better Business Bureau. There's even an alert on the BBB site saying that clients should be aware of possible data leaks at Quest. Not good, folks. When you jump over to Trustpilot, the results aren't any better, with an average rating of just 1.6 stars out of 5. Complaints include difficulties reaching representatives when problems arise, results that are contradictory or inaccurate after further testing is done, and an overall experience that is stress-provoking.

Best as a last resort

Use Quest for your fertility testing if you must (if they're the only game in town as far as labs go, or you can't find the specific test you need with another provider). Just don't be shocked if it's not the most pleasant experience.

Expensive no matter how you look at it

As you can see, there are options for both men and women with Labcorp on Demand. But, you may also see, when you compare the offerings here with rival providers on our list, that there's nothing truly comprehensive that incorporates multiple dimensions of fertility. You'll have to cobble together several tests to cover them, and that's going to get expensive fast. Also, these tests are often priced much lower on competitor sites, so in both ways you'll pay more for fertility testing at Labcorp. The one upside is that you can use your HSA/FSA to pay for most tests that Labcorp offers.

At home or in the lab? That will depend on the test. While you might think that Labcorp would require all sample collection to be done in their labs, a few options allow for discreet home testing too. You'll find all of those details on each fertility test when you click on it.

Can take up to 2 weeks to get results

We were surprised at how long it takes for Labcorp's test results to come back: evidently, it's the tests themselves that are "on demand”, and not so much for the timeframe for processing the samples. For example, the Ovarian Reserve test (AMH, FSH, Estradiol) tells you to expect results in 7-14 days from "when your sample arrives at the lab” (which is not the same as the day you visit Labcorp for the blood to be drawn). Those results can be viewed within your Labcorp Patient account; if there are any results that require a prompt response, you'll also get a phone call or notification via regular mail.

No physician consultation included

Either way, you'll want to follow up with your doctor. Labcorp on Demand doesn't include any kind of physician consultation on your fertility testing, taking their service down another notch when compared with some of the options we evaluated.

The final straw for us was Labcorp's reputation. There were more than 500 complaints filed with the Better Business Bureau over the last three years, and the way that Labcorp handled them was poor enough to earn a "C-” from the BBB. Plus, the clients that left ratings (and not just complaints) were equally unhappy, averaging just 1.3 stars out of 5. Most of the complaints we found were regarding billing, which likely doesn't apply to the on-demand fertility testing you're considering. Still, it's an indicator that you could be in for a hassle with Labcorp.

No good reason to buy fertility tests here

We struggle to find a compelling reason to choose Labcorp on Demand for fertility testing. They don't have as many types of tests as some of their rivals provide, they're more expensive for almost every single one that they do offer, and the results will probably take much longer to arrive.
